Requires an appointment — submit the Pet Surrender Request Form (the only way to be considered). An Intake team member follows up to discuss the situation, alternatives, and scheduling. Owner must be present with valid ID; surrender fees are due at the appointment. A 90-day waiting period applies before adopting again after a surrender.
